Weight is not scored by the number on the scale. What matters clinically is whether it has begun to affect anything yet — blood pressure, blood sugar, the liver, your knees, your day.

That is what a staging system does, and there is a published one: the Edmonton Obesity Staging System. Indian standard lipid panels that include either marker

In range is not the same as healthy.

Your lab prints a reference range. It does not print your target. A value can sit comfortably inside the range printed on the page and still be well above the level associated with a longer life — and nothing on the report will tell you that.

The two markers below are the clearest example. Neither appears on a standard Indian lipid profile. Both are cheap. One of them you only ever need to measure once.

ApoB

The number of cholesterol-carrying particles in your blood, rather than the amount of cholesterol inside them. Two people with identical LDL can carry very different particle counts, and it is the particle count that lodges in artery walls. Guidelines now set targets under 55, 70 or 90 mg/dL depending on your risk.

Lipoprotein(a)

Inherited, largely fixed for life, and unusually common in South Asians. It is measured once and never needs repeating. High Lp(a) does not mean something has gone wrong with your lifestyle — it means everything you can change now matters more than it would for someone without it.

South Asians develop coronary disease around a decade earlier than most other populations, and often without the classic warning signs. Lipoprotein(a) is one of the better-documented reasons — there is a substantial body of published Indian cardiology research on it as an under-recognised driver of premature heart disease.
